• Assessed the care needs of my residents and developed a comprehensive care plan that focused of the needs and wishes of the residents and their nominated representatives.

• Oversees that all 36 residents were safe during the nights and that all their routine care needs were met and handed over to morning staff ensuring that accurate information was carried to staff to enhance continuity of care.
• Assesses the wounds of residents, dress wounds, changes nephrostomy and colostomy site dressings. liaises with TPN, and meticulously implement the review to attain optimum results.
• Assessed the MUST scores my residents, discusses changes with management, makes dietician referrals and implement intervention. Refer interventions to GP and as well prescription request when required.
• Performs post fall assessments, update care plan and refer residents for physio and GP assessment, transfer residents for acute care when required .
• Carefully listens to concerns and complain from families, acknowledges them and offer support, follows the facility’s complaint protocol when required.

Post Operative management

• Received patient from PACU, oriented them to the ward environment and implemented consultant protocols as required.
• Assessed the pain scores of patient, administered pain meds such as opioids to ensure patients were comfortable and monitored for for opioid toxicity
• Performed neuromuscular assessment for patient who went through different forms of ortho surgeries and escalated changes promptly.
• Wound assessment and dressing , removed drains, collaborated with tissue viability nurse when required and carefully implemented plan.
• Identifies high risk and deteriorating patients and escalates for timely intervention.Performed their INEWS and ABCD assessments and promptly escalates.

Client was a 79 year Old Man who was diagnosed DM Type 1, Hypertension, Senile Dementia, left hip dislocation and fracture with hip replacement done.

• Created a safe environment with the support of the family to prevent falls and accidents, removed hazards and used used bed with side rails to prevent fall.
• Regularly assessed the patient's cognitive function, physical health, and emotional well-being reported back to the team on daily basis.
• Adopted the use of clear simple and simple language while communicating, and was always patient and allowed client to process information and respond.
• Informed the family on the risk of pressure sores, ripple mattress was secured. Assessed client for bed sores, applied barrier creams and family were taught how to change the soiled line in, diapers in my absence. carefully turned client to the non affected part and suppported with soft pillows.
• Engaged the patient in his hobbies to stimulate cognitive function and provide a sense of purpose. Always discussed patient childhood football club where he played, watched their matches occasionally and got him a jersey of the club.
• Arranged for a dietician and physiotherapist with support of the family to ensure client received proper nutrition, hydration, exercise. Patient diet plan was carefully explained to the house aide.
• Prepared client to the hospital on review dates, updated the family on the review and progress and always reassured my client and family.
• Client incisional site was always assessed, dressed aseptically and pictures and progress report given to the surgical team weekly before the review day.
• Client FBS/RBS and vital signs were always monitored twice daily and values document on the vital chart. All patient medidcations were administered and monitored for responses and progress. House help and son were taught how to administer patient oral medication and long acting insulin and document on the insulin chart over time.
