  • Created a safe environment with the support of the family to prevent falls and accidents, removed hazards and used used bed with side rails to prevent fall.
  • Regularly assessed the patient's cognitive function, physical health, and emotional well-being reported back to the team on daily basis.
  • Adopted the use of clear simple and simple language while communicating, and was always patient and allowed client to process information and respond.
  • Informed the family on the risk of pressure sores, ripple mattress was secured. Assessed client for bed sores, applied barrier creams and family were taught how to change the soiled line in, diapers in my absence. carefully turned client to the non affected part and suppported with soft pillows.
  • Engaged the patient in his hobbies to stimulate cognitive function and provide a sense of purpose. Always discussed patient childhood football club where he played, watched their matches occasionally and got him a jersey of the club.
  • Arranged for a dietician and physiotherapist with support of the family to ensure client received proper nutrition, hydration, exercise. Patient diet plan was carefully explained to the house aide.
  • Prepared client to the hospital on review dates, updated the family on the review and progress and always reassured my client and family.
  • Client incisional site was always assessed, dressed aseptically and pictures and progress report given to the surgical team weekly before the review day.
  • Client FBS/RBS and vital signs were always monitored twice daily and values document on the vital chart. All patient medidcations were administered and monitored for responses and progress.
