Regularly assessed the patient's cognitive function, physical health, and emotional well-being reported back to the team on daily basis.
Adopted the use of clear simple and simple language while communicating, and was always patient and allowed client to process information and respond.
Informed the family on the risk of pressure sores, ripple mattress was secured. Assessed client for bed sores, applied barrier creams and family were taught how to change the soiled line in, diapers in my absence. carefully turned client to the non affected part and suppported with soft pillows.
