• A 120 bed hospital with emergency services,medical ,surgical and Peads ward,with separate Gyne&obs facility.
• My clinical routine includes attending emergencies :Trauma,Cardiac emergencies and non cardiac semergencies .
• Triage and managing them by stabilizing them,maintaining IV lines and focused history taking is my responsibility.
• Preliminary treatment plan is crucial and usually made by us ( ER team) and then communicated to staff nurses in ER,lab and scans team.
• On call induction of cardiac and trauma teams is also part of plan as per cases
• Besides thromblising ,SK administration,spliniting ,wound management,stitching wounds and scans preparation is my responsibility.
• My task is to manage triage room and ER ward maintenance for incoming and outgoing patients.
• Documentation and communicating with ER and Trauma team is essential part of daily routines.
• Rota management and stress management for ED doctor's and staff
• Active and passive learning is crux of my daily clinical practice.
• Clinical tutor:Management lectures on ED cases,History taking and red flags in ER on weekly basis to junior fellows.

• It is a 50 bed hospital facility in multan and a chain of hospital that works on referal system.
• It consists of a tertiary care hospital and secondary level hospital and set of dispensaries.
• I was posted for almost two year tenure in Sahiwal,Dispensary.
• During my tenure there i was the front force to treat and manage COVID -19
• My daily routine involved out patient medicines and Emergencies.
• I was managing Chronic patients such as CLD,IHD,DM who were attacked by COVID-19.
• Starting from ABCD approach ,under strict COVID protocols treating them on admission basis.
• Sending labs,managing meds and complications and in case of deterioration managing ambulance evac to tertiary care hospital lahore for further managemnet.
• Our hospital proudly diagnosed more than 50 patients of covid on clinical symptoms later confirmed by PCR.
• Our X-RAY department helped me alot in time of lockdown to screen patients on the basis of XRAY and sore throat or URI presentations.
• Before UK hospitals ,we as a prophlactic appraoch used DEXAMETHASONE for patients in IM form to help fight covid.
• Besides this managing the team for screening purpose was my responsibility.
• Participated in Medical audit there.
• Provided WHO TEAMS with necessary data there.

• It is a 150 bed cancer hospital. One of the most prestigious hospitals in Pakistan.
• My job description was to scheduled the diagnosed medical oncology patients for chemotherapy.
• pre-chemo labs and address to the active complaints such as fever,nausea and vomiting or even sepsis.
• Managing complications and quick labs such as ANC and Lfts .
• Consults with on-call fellows there for patients complaints followed by in door rounds and ward rounds.

Learning pearls=
My consultant trained us in managing and triage Eclampsia and preeclampsia because this was the major cause of death in first time checkups of patient coming from deprived areas or those with obesity.

Clinical Tutor:

Daily rounds and lectures on ER cases and management

•It is a 1196 bed hospital divided into 4 medical wards along with other departments.

Responsibilities covered indoor and outdoor patients as well as two emergency duties per week. My job was full time
, regular and residential.

• History taking, physical examination and case presentations during ward rounds and in classrooms
• Managed outdoor patients like having infectious diseases, STDS, allergies, GERD, Diabetes Mellitus, Hypertension etc
• Managed patients of CLD with complications, Stroke, CRF with complications, Congestive heart failure, COPD etc in the ward.
• Did 30 days rotations in liver clinic.
• Dealt with emergencies like chest pain, arrhythmias, acute severe asthma, epilepsy, diabetic coma, drug overdose and poisoning etc.
• Performed procedures like I/V Cannulation, Foleys Catheterization, Lumbar Puncture, Paracentesis, N/G Intubation and Arterial Puncture.

Responsibilities:

• Receiving patients in Eye department either as Acute cases or Out patient. Taking full history as well as doing full clinical examination after stabilizing the patient
  Performing Eye examination ,labs,consults and diagnosis. Inserting IV lines and taking blood samples for appropriate lab investigations.
  Shifting patient to procedure room for foreign body removal and treatment.

My responsibilities are to take care of the patients with various surgical issues in surgery ward .

• I am covering indoor duties and my job position is full time and regular.

• History taking, clinical examination and case discussion with registrar and board round..

• Learning to co-operate involving multi team approach and performed duties in outpatient department.

• I learned preoperative care and post operative care of routine and emergency surgical cases.

• I covered the surgical emergencies by attending Road traffic accidents, burn cases, Diverticulitis, Acute Intestinal Obstruction, Perforated Peptic Ulcer, Acute Appendicitis, Gangrene, Head Injuries, plus all surgical cases of acute nature.

I have done surgical procedures like wound Debridement and care , Chest Intubation, , Appendectomy and other minor procedures under supervision of my consultants and assisted them in many type of major surgeries.

• During my day to day routine/work I also perform procedures such as suturing the wounds, nasogasteric tube insertion, backslabs and foley's catheterization.
