Community Liason Nurse

  • Management of an assigned caseload of community residents (20 currently)
  • Working alongside GP to manage service users needs and staff's concerns regarding same.
  • Triage GP referrals received from both campus and community.
  • Develop and complete health assessments and annual medical reviews in line with the regulations of the Health Act in addressing existing and emerging health needs for all residents on caseload.
  • Provide nursing education and advice to residents and non nursing staff regarding changing healthcare needs.
  • Support referrals and follow up to GP, Clinicians, nurse specialist services and outpatient departments.

  • Provide education for residents, direct carers and their families on developed healthcare plans and guidelines.
  • Efficient and timely handover of care to nurse managers and other clinicians.

  • Support residents with person centred needs in identifying requirements for and attending screening programmes and diagnostics e.g. National screening programmes, phlebotomy, bone density, bowel screening.

  • Vaccination program - I was part of the vaccination program for both service users and staff in Stewart's care.
  • Promoting person-centered care during transitions from ID settings to acute hospitals, collaborating with interdisciplinary team. Making referrals where required
  • Liaising with external services i.e Epilepsy team, GP services and relevant members of MDT that are related to each individuals healthcare needs
  • Service user education related to their needs- Organized CNS to attend staff meeting and made easy read information for healthcare need.
  • Advocating for service users when in hospital and on discharge. Ensuring that the service user is discharged with all safety measures in place prior. Any adaptations that are required prior to discharge are arranged.
  • De-sensitization programe- This was a programe that I played a part in developing for service users that required de-sensitization and skills building in order to engage in specific medical procedures related to their current health needs.
  • Capacity building - This is a huge part of my role currently. This is achieved by providing service users with easy read/video and additional supports that are required in order to effectively engage in healthcare to ensure better health outcomes. Also engaging with the National Advocacy Service when the service user may need an external advocate depending on health need.
  • Hospital passport- updating hospital passports and ensuring that medical diagnosis, history and current needs are reflected in this. Along with other information: communication, mobility,medication and any current healthcare needs.

Respite care

During this role I have nursed, cared, and supported individuals who have intellectual disabilities when they experience both physical and mental ill health. 

This includes liaising with the multidisciplinary team and clinicians where appropriate. Respite provides support to over 90 individuals (Children and adults) and their families. - During my time as working as staff nurse, I supported with management in the overall running of the respite unit which oversees the delivery of care. - As part of my duty of care I assess, monitor, and record any changes or symptoms in service user condition to the relevant clinician. - Monitor and observe all aspects of service user care including diet and physical activities. - I Support individuals attending respite and their family to ensure that they have access to the supports that are appropriate to their individual needs not only while in respite but at home also. - I support individuals with complex care needs that are both physical and psychological. - I implement appropriate assessment tools and recording sheets for the above mentioned to ensure that appropriate action is taken in the management of the individuals current need. The needs of individuals could present in Behavioural, Mental ill health and Physical Health needs that are contributing factors behavioural presentations. - Shift leader: throughout the shift I have delegated to staff to ensure that all roles throughout the day are completed. If staff are not competent/confident or do not have the skills that are required to complete task I will ensure that they have opportunity to participate in training to gain competence. I have provided staff team with articles based on evidence-based practice throughout my time in Respite related to specific needs of service users. - Preceptorship: I have completed my preceptorship training and am therefore competent in supporting students while on placement in Respite. Maintaining a good learning environment for both students and staff is important as it ensures that as a team we are continuously learning and maintaining standards and best practice. - I have ensured the service exceeds quality and health and safety requirements. - I worked as part of a multi-disciplinary team to support the reopening of respite following the closure during the pandemic. This involved the review and updating of all systems in respite to ensure we were operating to clinical governance and risk management requirements. - Ensure that medication management was always adhered to ensure that safe administration of medication as per medication management policy and most current legislation. - I had to adapt quickly to a new working environment and was very flexible with my working hours taking on overtime, working weekends, and doing night duty. It was a challenge but something that I enjoyed doing. It allowed me to see how resilient I could be when faced with such challenging times. - Ensure that a safe, secure, and person-centred environment, where service users receive the appropriate care, support, and treatment to manage their complex and long-term needs. - Utilise leadership, communication and delegation skills and effectively build positive relationships with service users, family members, clinicians, and other medical professionals. - Promote teamwork to develop a positive working environment which motivates all staff members. - Prepare the service to be compliant with HIQA regulations. always Ensuring we regulate to safeguard vulnerable individuals. Responsible for submitting all required notifications. - Deliver all aspects of nursing care in conjunction with the multidisciplinary team to ensure and achieve a high standard of holistic care and optimum health for service users. Ensure all nursing care is prioritised, provided, and delivered in a collaborative, confident and conscientious manner. - Advocate daily on behalf of service users where needed and empower service users by delivering health education and nursing care that enhances wellness and quality of life. - Manage resources efficiently, complete daily administration duties and provide a service that is cost effective to services users and their families. - Manage the provision of individual services and develop individual timetables specific to service users' needs and abilities.
