My personal and professional experiences have instilled in me valuable life skills such as empathy, patience and adaptability, which I apply in all my interactions. I’ve always been drawn to paediatrics because of my genuine love for working with children and their families. The chance to support and nurture young lives during their formative years is incredibly fulfilling for me. I find great joy in being part of a team that helps children grow and flourish, and in providing families with the care and support they need during critical times.

Child & Adolescent Mental Health service

9th January - 17th February 2023

I observed comprehensive assessments of children and adolescents to identify mental health needs and occupational performance issues. 

I collaborated with clients, families, and the multidisciplinary team to create personalised intervention plans aimed at enhancing mental health and daily functioning. 

I facilitated individual and group therapy sessions using evidence-based approaches to support emotional regulation, social skills, and daily living activities. 

I advocated for the needs and rights of clients within the multi-disciplinary team I maintained accurate and detailed records of client progress, treatment plans, and outcomes to inform ongoing care and professional practice.

Stroke Rehabilitation Ward

15th May - 23rd June 2023

I performed detailed assessments of stroke patients to determine functional deficits and therapy needs. 

I collaborated with patients, families, and the multidisciplinary team to create and execute individualised rehabilitation plans focused on improving mobility, cognitive function, and activities of daily living. 

I led individual therapy sessions utilising evidence-based practices to enhance motor skills, coordination, and cognitive abilities. 

I actively participated in multidisciplinary team meetings to discuss patient progress, share insights, and coordinate comprehensive care plans.

I provided education and training to patients and their families on adaptive techniques and the use of assistive devices to promote safety and independence at home. 

I maintained accurate and detailed patient records, progress notes, and treatment plans to ensure continuity of care and support clinical decision-making.

Community Rehabilitation, 

16th October - 15th December 2023

I evaluated clients' living environments to identify barriers and recommend modifications to enhance safety and accessibility.

I collaborated with clients, families, and healthcare teams to create tailored rehabilitation plans focusing on improving daily living skills and community participation. 

I provided one-on-one therapy sessions aimed at improving clients’ physical, cognitive and psychosocial functions. 

I actively participated in multidisciplinary team meetings to discuss patient progress, share insights, and coordinate comprehensive care plans. 

I educated clients and their caregivers on adaptive techniques, assistive devices, and strategies to manage their conditions and improve quality of life. 

I maintained detailed and accurate records of client assessments, progress notes, and treatment outcomes to support ongoing care and program evaluation.

Frailty Ward

19th February - 26th April 2024

I assessed elderly patients to identify physical, cognitive, and psychosocial needs, focusing on maximising their functional abilities. 

I created individualised treatment plans in collaboration with patients, families, and the healthcare team to address mobility, self-care, and safety concerns. 

I actively participated in multidisciplinary team meetings to discuss patient progress, share insights, and coordinate comprehensive care plans. 

I provided education and training on adaptive techniques, assistive devices, and fall prevention strategies to patients and their caregivers. 

I assisted in planning for safe discharge, including home modifications, community resources, and follow-up care to ensure continuity of care. 

I maintained accurate and detailed patient records, progress notes, and treatment outcomes to support clinical decision-making and quality care.
