• Conduct thorough patient assessments to detect changes in condition and implement appropriate interventions.
• Works on discharge planning by coordinating follow-up care ,arranging home health services,and providing patients with the necessary information for there post discharge recovery.
• Early detection and monitoring health changes for post operative patients.
• Provide high-quality care to patients with acute and chronic conditions
• Administer medications,IV fluids and other treatments as prescribed by GP.
• Utilizing specialized equipment such as cardiac monitors,defibrillators and ventilators.
• Communicate effectively with patients and their families,providing education and support regarding patients conditions and treatments.
• Delivering specialized care for patients with acute coronary syndrome,heart failure, myocardial infractions and other critical conditions.
• Continuously monitor and record patients vital signs including heart rate,blood pressure,respiratory rate oxygen saturation. Utilize advances cardiac monitoring systems to track and intercept electrocardiograms and other parameters.
• Perform and assist in emergency interventions.
• Develop and implement individualized care plans based on comprehensive assessments and patient needs.
• Evaluate physical,emotional,psychological status to determine appropriate care interventions.
• Assisting in obtaining and interpreting diagnostic test including lab work,ABG,X-rays,echocardiograms.
• Work closely with a multidisciplinary team including cardiologists,surgeons,anesthesiologists,occupational therapists,respiratory therapists and dietitians to develop and execute comprehensive care to patients.
• Educate patient and their families about conditions,treatment plans,medications,lifestyle modifications.
