• Employed on a casual, part-time basis, training pharmacy students in the Practice of Pharmacy through tutorials and workshops.
• Also facilitated Interprofessional Learning sessions between medicine and pharmacy students at the Mercy Hospital, Cork, as part of this role.

• Reviewing inpatient prescriptions, reconciling medicines lists, medicines optimization and patient counselling.

• Provided clinical pharmacy services to the paediatric ward, paediatric Day Stay Unit and paediatric outpatient clinic.
• Screening inpatient prescriptions, supporting medical and nursing staff by answering queries relating to the use of medicines in children (dosage, administration, safety), assisting paediatricians in writing funding applications for specialist drugs, counselling patients and their carers on safe and appropriate use of medicines.

• Provided clinical pharmacy services to adult medical patients on the Admission Unit, General Medicine, Respiratory, Geriatric and Orthopaedic wards.
• The role included medicines reconciliation, prescription review, patient education, as well as supporting nursing and medical staff with medication related queries.
• Operational areas included the Dispensary – ensuring safe supply of medicines to inpatients.
• Education – mentoring intern pharmacists; orientation of new medical staff at the hospital.
• Guideline Review – review of medication related policies and protocols.

• Provided pharmacy services to the Admission and Planning Unit and the Emergency Department, Neurology and Neurosurgical wards.
• Also worked on Geriatrics, Intensive Care Unit and cardiology during this time.

• Developed and piloted a discharge pharmacy service on the general medicine wards which led to the funding of a dedicated discharge pharmacist at the hospital.

• Clinical screening of chemotherapy prescriptions and provision of inpatient pharmacy service on the oncology ward.

• Central to this role was the implementation of PHARMAC’s Hospital Medicines List (HML).

* PHARMAC, the Pharmaceutical Management Agency, is a New Zealand Crown entity that decides which medicines and pharmaceutical products will be subsidized for use in the community and in public hospitals.

-Collating organisation-wide feedback in response to PHARMAC consultations.

- Acting as liaison person between PHARMAC and ADHB.

- Communicating changes to the Pharmacy Department, the HMC and Clinical Directors.

- Conducting teaching sessions and updating electronic resources to reflect formulary changes.

Other regular duties as Secretary of the Medicines Committee included;

- Facilitating access to high-cost medicines at ADHB;

- Organising HMC quarterly meetings;

- Presenting drug usage reports to the HMC;

- Maintaining electronic archives and databases for HMC meetings and decisions.

• From January 2013 to August 2013, I assisted in the role of procurement, working closely with inventory staff and communicating with suppliers regularly to facilitate access to medicines at ADHB.
• Coordinated policy and guideline review within the Auckland Hospital Pharmacy Department from September 2012 to September 2013.
• From June 2015 to October 2015, reviewed and updated the ADHB Medication Administration Guidelines.

• Hosting third year pharmacotherapy workshops.
• Subsequently invited back as guest lecturer for neurology (epilepsy and Parkinson’s disease) in 2012 and 2013.

• Clinical pharmacist in the Emergency Department, with medicines reconciliation of all newly admitted patients, and clinical pharmacist for the neurosurgical ward.
• Also worked in the clozapine clinic, geriatric ward, cardiac rehabilitation, oncology compounding unit and dispensary.

• Full-time locum pharmacist with Horgan's retail pharmacy group, working at branches across Cork county.

• Sole pharmacist in a busy retail pharmacy, working with a team of six technicians and assistants.
